1.1 Wellbeing Elements Pty Ltd (ACN 681 940 714) (we, our, us) and its associated entities are

addresses, telephone numbers, booking details and membership information.

by email, via our website www.eq-wellbeing.com, through online booking, membership and
payment platforms (including Momence), through digital intake forms, waivers and
questionnaires, through print or digital media, via cookies and similar technologies, and from
third parties where permitted by law.
2.3 We collect Personal Information for purposes including providing our services, managing
bookings and memberships, ensuring services are appropriate, communicating with clients,
processing payments, maintaining safety and legal compliance, and marketing.
2.4 We may also use Personal Information for secondary purposes closely related to the
primary purpose where you would reasonably expect such use or disclosure.
2.5 Where you have consented to receive marketing communications, consent remains until
withdrawn.
2.6 You may unsubscribe from marketing communications at any time using provided
unsubscribe functions or by contacting us.
2.7 We may use automated systems to send booking confirmations, reminders, updates and
marketing messages via email or SMS.
2.8 When collecting Personal Information, we will explain why it is collected and how it will be
used where practicable.
3. Sensitive Information
3.1 Sensitive information includes racial or ethnic origin, political opinions, religious beliefs,
trade union membership, criminal record or health information.
3.2 We may collect health information through digital intake forms, waivers, acknowledgements
and health screening questionnaires administered via booking platforms including Momence to
assess suitability and meet safety and legal obligations.
3.3 Sensitive information is collected and used only for the primary purpose, directly related
secondary purposes, with consent, or where required or authorised by law.
4. Third-Party Service Providers
4.1 Where reasonable and practicable, we collect Personal Information directly from you. In
some circumstances, information may be received from third parties.
4.2 We use third-party service providers including Momence for booking, membership
management, waiver administration and payments. These providers may collect, store and
process Personal Information in accordance with their own privacy policies.
4.3 We take reasonable steps to ensure third-party providers handle Personal Information
consistently with the Australian Privacy Principles.
5. Disclosure of Personal Information
5.1 Personal Information may be disclosed to third-party service providers (including
Momence), payment processors, with consent, or where required or authorised by law.
5.2 Some third-party providers may store or process Personal Information outside Australia,
including in the United States. Reasonable steps are taken to ensure overseas recipients
comply with the APPs.
6. Security of Personal Information
6.1 Reasonable steps are taken to protect Personal Information from misuse, loss,
unauthorised access, modification or disclosure.
6.2 Information may be stored electronically via secure systems or in physical records
accessible only to authorised personnel.
6.3 When no longer required, Personal Information will be destroyed or permanently
de-identified.
6.4 Records may be retained for a minimum of seven (7) years where required by law or for
legitimate business purposes.
7. Access to Your Personal Information
7.1 You may request access to or correction of Personal Information we hold. Requests must
be made in writing and will be responded to within 30 days.
7.2 No fee is charged for access requests, though a reasonable administrative fee may apply
for copies.
